Well, this very largely depends on the tone of voice my friend is using. If she/he is going through a bit of a traumatic time, I will probably take it to mean they need to talk about their situation.
But, if this isn't the case, and/or they say it with slight hesitation to their voice, or indeed, they sound slightly annoyed/angry, I do get anxious, and immediately say out loud or to myself, "oh oh!" followed by "what is it", automatically jumping to the conclusion that it's something to do with me, ie. I've been a bad friend etc.

I'm the same way. Even if the other person's tone isn't necessarily expressive, my first thought is usually: Oh man, what did I do now? I immediately then feel bad/guilty.

My reaction to that question nowadays is "where the heck can I fit them into my Outlook Calendar?" It has become more of a question of "when" to see them versus "Why"
